In-home Supports

New Communities' In-Home Support program is designed to support adults, in their home setting, to be active participants in their lives. Services focus on enhancing skills through supported daily living, behavior management, communication, socialization, and community based activities. Our team works one-on-one with clients to develop plans focusing on goals and objectives, specifically tailored to each individual's needs and desires. Our in home support team is committed to maximizing client independence and success in the community while, at the same time, ensuring the health, safety and well-being of the client.

Dedicated and Highly Trained Staff

New Communities' staff are trained and certified Direct Support Professionals that provide services in a variety of residential settings including a client's own home. Our team provides services aligned with the individual's approved plan and are able to administer and monitor all client medications, coordinate medical appointments and work and day programs, and ensure appropriate transportation. The New Communities' team works diligently to support clients in being involved and active members of their community.
